Download the Application for Admission form and the Personal Information Sheet from the links below and print them on size A4 bond paper.
Accomplish forms using your own handwriting. Submit filled-up forms with the following:
a. Photocopy of your final, completed Grade 9 report card (SY2015-2016)
b. Photocopy of your Grade 10 report card for the 1st Grading Period (SY2016-2017)
c. HANDWRITTEN autobiography on size A4 bond paper (Important: In order to help the Committee on Admission and Aid make a judgment on the strand you are best suited for, please highlight your special aptitudes/abilities and interests in your essay.)
d. Two recent I.D. pictures (1” x 1”) in COLOR
e. Photocopy of Birth Certificate (NSO Copy)
– If applicant is NOT a Filipino citizen, submit a photocopy of your
i. Alien Certificate of Registration (ACR), and
ii. Special Study Permit issued by the Bureau of Immigration.
If applicant has DUAL citizenship, submit a photocopy of your Philippine Passport or a Certificate of Recognition as a Filipino Citizen.
f. Two Letters of Recommendation from the last school attended (The persons recommending you may choose to submit recommendation letters OR may fill out the Recommendation Form that can be downloaded from the Ateneo website.)

Hi may I ask if yung Xavier Ateneo shs enyrance exam ay similar din sa ashape? They say xavier is the closest to admu among all loyola schools. But ashape is harder than xavier ateneo shs entrance exam. Do you guys know what topics shoild I review and where to focus more?

The exam schedule vary depending on the school. Usually, entrance exams for senior high are administered starting from September onwards.
